In order to obtain the aid, it’s necessary to submit a written application to the Pantelis Boumbouras – President of the Foundation and provide copies of the confirmation documents.
The application should specify:
- surname, name and patronymic;
- full postal address (with index);
- contact information (e-mail, phone number)
- if it concerns a child, indicate his / her name, date of birth;
- diagnosis, condition, cost of treatment (confirmed with copies of relevant certificates);
